Student Athlete Registration

Complete all 3 steps to be cleared for participation.

1. Online Registration

• Complete all digital forms via FinalForms (Parents and students must sign together).

• Need help? Review the FinalForms Setup Guide.

2. Sports Physical

• A valid Sports Physical Form PDF must be on file with the Athletic Office.

• Valid for 24 months and must cover the entire upcoming season.

• Upload directly to FinalForms or turn it into the Athletic Office.

3. Fees & Fines

• Pay all fees online via the PASD Online Payment Portal or in person at the ASB Secretary's Office.

• Required Fees:
  • ASB Card: $5 (Required annually)
  • Annual District Fee: $50 (One-time annual fee)
  • Seasonal Fee: $25 (Due per sports season)

  • Cap: Individual student fees are capped at $100 maximum per year

• Contact the Athletic Office to request a fee waiver.
